Eligibility Criteria

Physician order and plan of care

Physician order and plan of care

A doctor (or authorized provider) must issue an order and establish a plan of care, reviewed periodically.

Face-to-face encounter

Physician order and plan of care

The patient must have had a face-to-face visit with their physician or authorized provider within 90 days before or 30 days after starting home health care.

Homebound status

Homebound status

The patient must have considerable difficulty leaving home without assistance or supportive devices. Occasional absences for medical care, religious services, or short outings are allowed.

Need for intermittent skilled care

Need for intermittent skilled care

 The patient requires skilled nursing or therapy (physical, occupational, or speech) on an intermittent basis.

Eligibility is confirmed through collaboration with the referring provider, ensuring all criteria are met in accordance with Medicare guidelines.
